Summary

This study is testing whether a combination of tools can help cancer survivors living in rural areas better manage their health after treatment ends. Participants create their own survivorship care plan and some receive a phone-based education session. The goal is to see if this approach improves survivors' knowledge and confidence, and helps their doctors provide better follow-up care.
