First seen Jun 24, 2026 · Last updated Jun 27, 2026 · Updated 1 time
Summary
This study looks at how to best help primary care clinics adopt team-based home blood pressure monitoring. Over 5,700 patients and their care teams will use a program where patients send in their home readings and get support from doctors, nurses, and community resources. The goal is to see if these strategies improve blood pressure control and reduce differences between Black and White patients.
