Summary
This study tests an online portal called SCP-PHR designed to help colon and rectal cancer survivors manage their survivorship care. Researchers will enroll 250 adults who have completed initial treatment within the past year. The goal is to see if the portal improves how often survivors get recommended surveillance tests and preventive care.
