This study looks at whether a combination of nurse health coaching, doctor training, and subsidies for a kidney-protecting drug can help slow the worsening of chronic kidney disease. About 724 adults with moderate-to-severe kidney disease in Singapore will be randomly assigned to receive this extra support or usual care. The main goal is to see if kidney function declines more slowly over three years in the group getting the extra help.
