Summary

This study will test whether measuring blood pressure at home and having weekly phone meetings with a pharmacist can help control high blood pressure. About 420 adults from two emergency department clinics will take part. Half will use the home monitoring and telehealth program for 6 months, while the other half gets usual care. The goal is to see if this approach leads to better blood pressure control.
