Summary
This study looks at whether a personalized exercise program, started in the hospital and continued at home with video supervision, can help older adults recover after being hospitalized for pneumonia or a urinary tract infection. The goal is to see if this approach is safe, improves physical function, and reduces the chance of being readmitted to the hospital. 460 participants aged 65 and older (or 18-64 with chronic conditions) will be enrolled.
