Home visits aim to get patients 'transplant-ready'

Summary

This study is testing a home support program for people on the kidney transplant waitlist who are currently 'inactive' or at risk of becoming inactive. A team of nurses, occupational therapists, and handy workers will visit participants at home over four months to help them overcome physical, mental, and social barriers. The goal is to help more patients become 'active' on the waitlist and improve their overall health and readiness for surgery.
