Summary
This pilot study tests whether a pharmacist-led program using a continuous glucose monitor (CGM) can improve blood sugar control and healthy behaviors in 40 adults with type 2 diabetes. Participants will use a CGM for 12 weeks and compare results to a similar group that only used standard finger-stick testing. The study aims to see if the CGM approach leads to better A1c levels and more time in a healthy blood sugar range.
