Summary

This study is testing whether pharmacists can help people lower their risk of heart disease. Over 1,000 adults with conditions like high blood pressure, diabetes, or who smoke will work with pharmacists for up to six months. The goal is to see if this support leads to better blood pressure, cholesterol, and lifestyle choices compared to usual care.
