Summary

This study is testing whether cancer survivors who get a personalized referral to a nutrition and exercise program will adopt healthier habits. It's enrolling 306 people who have finished treatment for breast, prostate, or colorectal cancer but aren't currently meeting standard diet and activity guidelines. The goal is to see if this targeted support helps survivors eat more fruits and vegetables and become more physically active.
