Summary
This study tests a comprehensive, remotely delivered care program for people aged 40 and older with Long COVID who have ongoing heart or lung symptoms like fatigue, shortness of breath, or chest pain. The program includes medication review, mental health support, telehealth coaching, and a personalized 24-week exercise plan using a wearable sensor. Researchers measure whether participants can walk farther after 6 months, along with changes in quality of life, mental health, and heart function over two years.
