Summary
This study tests whether a new team-based approach can help primary care doctors diagnose anemia and decreased kidney function more accurately and quickly. The approach includes automated tracking of abnormal test results, adding care partners to guide follow-up, and using nurse navigators to involve patients. About 450 patients and their doctors across 13 clinics will take part, with some clinics using the new approach and others continuing usual care.
