New program aims to ease tough transition for teens with Crohn's

Summary

This study tests a program to help teenagers with Crohn's disease or ulcerative colitis move from pediatric to adult healthcare. About 90 teens aged 16 to 17.5 will either get the program (which includes a navigator, skills training, and education) or standard care. The goal is to see if the program improves their daily functioning and readiness for adult care over three years.
