Summary

This study looks at whether a program that combines remote patient monitoring with care management can help people with uncontrolled hypertension lower their blood pressure. About 130 participants will be followed for up to 24 months. The goal is to see if this approach can be used widely across a large healthcare system.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

remote patient monitoring and care management program

What this could lead to

If successful, this could show that combining remote monitoring with care management helps more patients control their blood pressure long-term.

What could go wrong

This is an observational study, not a controlled trial, so results may be influenced by other factors. It is also limited to one healthcare system, so findings may not apply everywhere.
