Summary
This study looks at whether a hybrid care pathway that includes home telemonitoring helps people with heart failure spend more days at home and out of the hospital. Participants measure their weight, blood pressure, heart rate, and symptoms daily, sending the data to a medical center that can intervene early. Researchers will compare outcomes for patients in the program with those receiving usual care across seven hospitals in the Netherlands. The goal is to see if this approach improves survival, quality of life, and medication use.
