Can primary care doctors improve cancer survivor Follow-Up?

Summary
This study looks at two new models of care for people who have finished treatment for early-stage breast or colorectal cancer. The goal is to see if these models help survivors get the recommended follow-up tests and preventive care they need. About 2,450 adults from Kaiser Permanente Southern California are taking part.
