Summary

This study tests a digital tool called DIPAR that sends reminders to adults with type 2 diabetes to get important lab tests, like kidney function and blood sugar checks. About 200 participants use the tool through a patient portal to see if it helps them stay on track with their care. The goal is to improve regular monitoring and disease management.
