Summary
This study tests whether helping people with diabetes find local resources (like food or housing help) and pairing them with a peer supporter can improve blood sugar and blood pressure. About 694 adults with diabetes who have trouble controlling their condition will be randomly assigned to different combinations of these supports. Researchers will track health markers and surveys over a year to see which approach works best.
