New hospital program aims to keep COPD patients out of trouble after Flare-Ups

Summary
This study tests whether a structured care program (called a transition bundle) helps people with COPD recover better after a serious flare-up. The program includes guided steps and regular check-ins. About 1,368 patients across 36 hospitals will take part. The goal is to see if this approach reduces the chance of their condition getting worse again.
