Summary
This study tested whether pairing people with a community health worker for 10 months could help them better manage chronic health conditions like diabetes, high blood pressure, and obesity. The program provided navigation help to access health services, education, and support for diet and exercise. Researchers compared this approach to usual care in over 200 adults from specific low-income neighborhoods.
