Summary
This pilot study tests whether a program of education, motivation, and social support can help adults with diabetes and early-stage chronic kidney disease (CKD) keep their kidneys healthier. Researchers will enroll 50 people and compare those who receive the program to those who get general diabetes information. The goal is to see if the program is practical and whether it improves patients' knowledge, confidence, and use of support services.
