Summary
This study is testing if a new clinic program can help control chronic diseases like high blood pressure, diabetes, and asthma. The program screens patients for social needs like housing or food insecurity and works to reduce racial bias in care. Researchers will enroll 68,000 adults and children from family medicine clinics to see if this approach leads to better health outcomes.
