Summary
This study aims to understand how older adults can maintain their health and independence as they age. It will follow 500 adults aged 50 and older in the Washington D.C. area for up to five years, giving them a detailed annual health check-up and a personalized 'Health Passport' with wellness tips. The main goal is to track health changes over time and see if the personalized recommendations help people improve their lifestyle and well-being.
