Summary
This study is testing a new toolkit designed to help primary care doctors and nurses have better conversations with people living with dementia about their future care wishes. It will compare the toolkit to usual care across 20 clinics to see if it leads to more and higher-quality discussions about goals of care. The research aims to improve how medical teams plan for the future with patients and their families.
