Summary
This study tests whether a personalized care plan at hospital discharge helps people who survived acute kidney injury recover better. Depending on their risk level, patients get education, a primary care visit, or specialist monitoring. The goal is to see if this approach increases the number of days patients are alive and out of the hospital.
