Summary
This study will see if community health workers (CHWs) can help people with two or more chronic health problems, like diabetes and high blood pressure, better understand and manage their health. The CHWs will work with 300 adults who have trouble getting good medical care to create a personal health plan. The goal is to see if this support increases patients' confidence and ability to handle their own health needs.
