Summary
This study will see if community health workers can help people with two or more chronic conditions (like diabetes, high blood pressure, or heart disease) better manage their health. Participants will work with a health worker to create a personal health plan and learn how to navigate the healthcare system. The goal is to improve patients' confidence and ability to take care of themselves.
