Summary
This study tests whether virtual group visits can help adults with type 2 diabetes and related heart conditions better manage their health. About 720 participants will join online group sessions led by healthcare providers. The main goal is to see if these visits lower blood sugar (A1c) and improve other measures like blood pressure and cholesterol.
