Summary
This study tests a 6-month program combining exercise training, problem-solving skills, and community health worker support for 210 adults with type 2 diabetes, obesity, and early signs of heart trouble. Participants must have low income and live in Baltimore or Washington County, Maryland. The goal is to see if this approach improves fitness and health better than just providing education and gym access.
