New program aims to get frail veterans moving after heart trouble

Summary

This study tests a face-to-face program called Transition to Cardiac Rehabilitation (T2CR) for Veterans with heart disease who also have other health problems or frailty. The goal is to help them build skills and motivation to successfully take part in home-based cardiac rehab. Researchers will compare those who get T2CR with those who receive usual care, looking at physical function, rehab participation, and days spent healthy at home over one year.
