Summary
This study is testing a proactive outreach program designed to help Black, Indigenous, and People of Color (BIPOC) who smoke to quit. Researchers will identify eligible patients through their health records and reach out to connect them with proven quit-smoking treatments like counseling and medication. The goal is to see if this culturally tailored, proactive approach is more effective than standard care at helping people quit smoking for good and improving their heart health.
