Summary

This study compares two telehealth programs—digital health coaching and remote patient monitoring—to see which better helps people with physical disabilities manage type 2 diabetes and other chronic conditions. About 5,000 participants will be followed for 12 months to measure changes in blood sugar levels and diabetes-related distress. The goal is to find the most effective approach for improving health in underserved groups.
