Summary
This study tests a computer alert that encourages doctors to provide or request palliative care for hospitalized patients at high risk of dying within six months. The goal is to see if this nudge helps patients spend more days at home and less time in the hospital. About 19,000 adults across six hospitals are taking part, and researchers will also interview doctors to understand what works best.
