Summary

This study tests whether having extra appointments with a heart specialist at a local GP practice helps people at high risk get better treatment for heart, kidney, metabolic, and lung conditions. About 138 adults from a previous study (FIND-AF) will either receive these specialist consultations or standard GP care. The goal is to see if the specialist visits lead to more people starting or adjusting medications to meet national guidelines, improving their overall health.
