Summary
This study tests whether a nurse-led education program can reduce hospital readmissions for patients with conditions like heart failure, COPD, or obesity who are having hip or knee replacement. About 300 participants will either receive the extra nurse support or standard care. The goal is to see if the program helps patients stay healthier after surgery and avoid coming back to the hospital.
