New Nurse-Led program aims to keep frail seniors independent after hospitalization

Summary
This study tests a program called INFA that helps frail older adults (age 65+) who are in the hospital for non-geriatric reasons. Nurses use routine checks to identify problems like falls risk, poor nutrition, or confusion, then create a personalized care plan with a team of specialists. The goal is to help patients maintain their ability to do daily activities like bathing and dressing six months after leaving the hospital. About 300 participants will be compared to those receiving usual care.
