Summary
This study tests a program called INFA that helps frail older adults (65+) who are in the hospital for non-geriatric reasons. Nurses use a special checklist to find health issues early and create a personal care plan. The goal is to help patients keep their ability to do daily tasks like bathing and dressing for six months after leaving the hospital.
