Summary
This study tested whether a home-based palliative care program could improve end-of-life care for people with serious heart failure. Researchers enrolled 709 patients to see if connecting them with specialized home care services would reduce hospital visits and costs while increasing hospice use. The program aimed to provide better comfort and support at home instead of frequent hospital stays.
