Summary

This study is testing whether using a special phone app with health devices and receiving personalized text messages can help people with chronic heart failure take better care of themselves. The goal is to see if these tools can help patients stay healthier at home and reduce trips to the hospital. Researchers will compare these digital tools against usual care in 360 participants over a year.
