Summary
This study is testing whether using a special mobile app with health devices or receiving personalized text messages can help people with chronic heart failure take better care of themselves. The goal is to see if these tools help patients stay healthier at home and reduce trips to the hospital. Researchers will enroll 360 participants to compare the app, the texts, both together, or usual care alone.
