Summary
This study tested a program to help 219 incarcerated individuals with HIV, hepatitis C, or substance use disorder transition from prison to community healthcare. Participants worked with a nurse case manager before and after release to plan and coordinate their care. The goal was to see if the program was feasible and acceptable, not to directly treat the diseases.
