Summary
This study tests a program that sends educational videos and messages through a patient portal to older adults with mild cognitive impairment or dementia and their care partners. The goal is to help them discuss and reduce medications that may be harmful or unnecessary. About 100 participants will be recruited from primary care to see if this approach encourages safer medication use.
