Summary
This study tests a program called 'Disfrutando' designed to help people with type 2 diabetes, especially those facing social challenges like food or housing insecurity. The program teaches practical self-management skills and addresses barriers to healthy living. About 31 adults with elevated blood sugar (A1C of 5.7 or higher) will participate to see if the program improves blood sugar control and weight.
