Inclusion Criteria: * Voluntary written informed consent of the participant or their legally authorized representative has been obtained prior to any screening procedures; * Adults 18 years or older; * Treated and followed-up for Type 2 diabetes at the department of endocrinology at the participating hospitals; * Patients with (the ability to access) the MyNexuz patient portal; * Patients with the ability to understand Dutch, French or English. Exclusion Criteria: * Participant is pregnant or hospitalized at the time of recruitment; * Are unable to use a mobile phone because of mental or cognitive impairment; * Deemed ineligible by the treating physicians.
