App aims to keep hearts strong after hospital rehab

Summary

This study is testing whether a digital support package helps people maintain their heart health after completing a standard cardiac rehabilitation program. It will compare the usual follow-up care to the usual care plus a digital program with educational modules, motivational messages, and activity tracking. The goal is to see if the digital tools help people better manage their blood pressure, cholesterol, activity, and mood over six months.
