Digital coach aims to keep hearts healthy after hospital

Summary
This study is testing whether adding a digital support package to usual care helps people maintain their heart health after completing a cardiac rehabilitation program. It will involve about 306 adults who have had a heart attack or heart failure. Participants will be randomly assigned to receive either standard follow-up care or standard care plus a digital program with educational modules, motivational messages, and activity tracking.
