App aims to keep kidney patients out of the ER

Summary
This study tests whether a special smartphone app (VIEWER) can help people with advanced chronic kidney disease (stage 5) or end-stage kidney disease stay healthier at home and avoid emergency room visits or hospital stays. About 340 adults will be split into two groups: one gets usual care, the other gets usual care plus the app to track their health. The goal is to see if the app reduces hospital visits and makes patients feel safer.
