App aims to keep kidney patients out of the ER

Summary

This study tests a special app and home monitoring system for people with advanced chronic kidney disease (stage 5 or end-stage). The goal is to see if using the app helps reduce emergency room visits and hospital stays compared to standard care alone. About 340 adults in Canada will take part, using the app to track things like weight and blood pressure at home.
