Summary
This study tested a program that helps seriously ill heart failure patients receive specialized care at home instead of in the hospital. Doctors were encouraged to refer eligible patients to a home-based palliative care service. The goal was to see if this approach reduces hospital stays, lowers costs, and improves end-of-life care. Over 700 patients participated, and the program aimed to make their final months more comfortable and less hospital-focused.
