Summary

This study aims to reduce heart-related health problems in Detroit and Cleveland by using community health workers to help people manage high blood pressure and prevent heart failure. About 364 adults with high blood pressure or other heart risks will receive coaching and support to improve their health. The goal is to see if this approach can lower blood pressure and reduce the chance of serious heart issues.
